Billing Information

In the State of Washington, physical therapists may treat patients without a prescription.

However, most insurance plans still require a referral or prescription for therapy written by one of the following healthcare providers. You must contact our office for an appointment within 30 days from the date of receiving a referral/prescription.

  • Physician
  • Nurse Practitioner
  • Chiropractor
  • Physician’s Assistant
  • Podiatrist

Cash paying clients do not need a physician's referral.

Coverage for physical therapy depends on your insurance company and your policy.

As a courtesy, we make every attempt to verify your eligibility, deductibles, co-insurance and co-pay amounts. However, we recommend contacting your insurance company to determine the specific coverage and benefits offered under your policy for outpatient physical therapy services.

At the end of each month, you will receive a statement from us showing the amount you owe, after we have applied any payments made by you or your insurance company. Benefit information we receive from your insurance company is not a guarantee of claim payment, and may be subject to any policy changes at the time of service.
