How to Schedule an Appointment

Scheduling is easy. You may call us directly at
385-1035 or stop by our office.

Please be prepared to provide information about:

  • Your Insurance Plan
  • Your Personal Contact Information
  • Your Injury
  • Your Referral

Please note that most insurance plans require that you have a referral or prescription for therapy from a:

  • Physician
  • Nurse Practitioner
  • Chiropractor (Medicare does not accept referrals to PT from chiropractors.)
  • Physician’s Assistant

This initial information allows us to verify your benefits with your insurance company. In most cases, we will be able to verify your therapy benefits in advance of your first visit.

What to Expect at Your First Visit

Please arrive for your first visit 15 minutes before your scheduled appointment time. Upon your arrival, you will be asked to complete a few forms. While we have limited the paperwork as much as possible, this process usually takes at least 10 minutes.

The receptionist will ask also you for the following:

  • Your insurance card
  • The referral or prescription
  • Any applicable co-pay
  • L & I Claim number (if applicable)

Once the paperwork is complete, you will be greeted by your therapist and taken back to a private examination room. You should expect to begin each visit without a significant delay. Your initial evaluation will consist of an interview and physical examination. Appointments typically last 45-60 minutes. Wear comfortable clothes and shoes.
